About the Opportunity
We are seeking an experienced Clinical Nurse Specialist Grade 2 – Respiratory Nurse to join the Murrumbidgee Local Health District nursing workforce. In this role, you will deliver advanced, patient-centred care for people living with chronic respiratory conditions, supporting improved health outcomes and helping patients remain healthier at home for longer.
Working closely with multidisciplinary teams, you will coordinate complex care across hospital and community settings, support safe transitions home, and provide expert clinical leadership, education and consultation to staff, patients and carers.
This is an excellent opportunity for a nurse passionate about chronic disease management, collaborative care and improving patient outcomes.
In this role, you will:
- Provide advanced clinical nursing care for patients with complex respiratory conditions
- Undertake comprehensive clinical assessments and develop coordinated care plans that support self-management and improved health outcomes
- Facilitate safe and effective transitions from hospital to home and support patients within the community setting
- Provide expert consultation, guidance and education to patients, carers and multidisciplinary teams
- Act as a clinical resource and mentor for nursing and allied health staff
- Lead and support evidence-based practice, service development and quality improvement initiatives
- Promote collaborative care across acute, primary and community health services throughout the district
- Advocate for patients and families while supporting innovative models of care that reduce avoidable hospital presentations

About You
Our ideal candidate will demonstrate:
- Current registration with the Nursing and Midwifery Board of Australia (AHPRA) as a Registered Nurse
- Postgraduate qualifications in respiratory, cardiac, chronic disease or related field (or working towards)
- Minimum 3 years full-time equivalent clinical experience relevant to respiratory, cardiac or chronic disease management
- Demonstrated ability to coordinate complex care and support patients managing chronic conditions across hospital and community settings
- Strong clinical leadership skills and ability to mentor and support staff
- Excellent communication, collaboration and care coordination skills within multidisciplinary teams
- Commitment to evidence-based practice, quality improvement and ongoing professional development
- Current Class C driver’s licence and willingness to travel across the district if required
