Our Cancer Care Centre is seeking to engage a Clinical Nurse Specialist in the role of Gastrointestinal (GIT) Cancer Care Coordinator on a temporary assignment. This position is responsible for improving the continuity of care, identifying issues, conducting psychosocial assessment, intervening early with attempts to avoid hospital admission or reduce length of stay amongst patient cohort for curative, adjuvant, and metastatic GIT oncology patients.

The successful incumbent will work closely with other Care Coordinators and Oncologist affiliated with Nepean Cancer Services. The role acts as the primary case manager for complex episodes ensuring the proper monitoring and evaluation of the patient’s care pathway in relation to service provision and complaints. What you will bring to the role:

  1. Current registration with AHPRA – Division 1 Registered Nurse.
  2. Post-registration qualifications in Cancer Nursing and at least 3 years’ experience working in Oncology, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
  3. Commitment to champion, educate and lead a multidisciplinary team to improve patients’ outcomes as part of a team.
  4. Demonstrated problem solving and decision-making abilities according to best practice.
  5. Demonstrated ability to communicate effectively with clients, carers, and a diverse range of health care providers.
  6. Demonstrated commitment to clinical practice development.
  7. Ability to work independently with minimal supervision maintaining involvement within the wider multidisciplinary team.
