As a key part of our team, you'll work alongside a Geriatrician to deliver expert advice and support to patients, families, RACFs, and NSW Ambulance teams. The role involves working alongside a Geriatrician to provide advice and support to patient /family/ RACF/ NSW Ambulance to reduce hospital presentations by providing a comprehensive geriatric assessment and management plan in collaboration with the multidisciplinary team as well as addressing acute patient deterioration.
What You'll be Doing
The COPILOT CNS2 will provide acute care outreach services to older persons in the community or Residential Aged care Facility (RACF) to avoid unnecessary Emergency Department presentations and will advise and support the patient/family/RACF/NSW Ambulance to reduce these hospital presentations by providing a comprehensive geriatric assessment and management plan in collaboration with the multidisciplinary team as well as addressing acute patient deterioration. They will also provide clinical leadership, support, and supervision for less experienced clinicians as well as the multidisciplinary team to achieve patient focused quality care.

- Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of the employees specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation
- Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
- Demonstrated experience and skills in providing complex care in the relevant field.
- Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced based nursing care.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
