About the Opportunity
We are seeking an adaptable and compassionate individual to join our supportive and collaborative team. Exciting opportunity to be trained in conducting Aged Care Clinical Needs Assessments for SWSLHD. If you have a passion for working with older persons, possess strong teamwork and communication skills, and have previously worked in the aged care sector, we encourage you to apply.
The clinical assessor provides a complex aged care assessment using the Integrated Assessment Tool (IAT) addressing the medical, physical, cognitive, psychosocial, cultural, environmental and social aspects of older people referred for assessment via My Aged Care.
Formulates a support plan that best meets the needs of the older person and their carer within the resources available.
What You'll be Doing
The clinical assessor provides a complex aged care assessment using the Integrated Assessment Tool (IAT) addressing the medical, physical, cognitive, psychosocial, cultural, environmental and social aspects of older people referred for assessment via My Aged Care.
Formulates a support plan that best meets the needs of the older person and their carer within the resources available.
Works within the guidelines set by the Single Aged Care Assessment System Contract and Statement of requirements, the My Aged Care Assessment Manual 2024 and the Aged Care Act 1997 and any amendments.

3. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted
