About the opportunity

No two days are the same. Work in a fast-paced environment where your clinical judgement, communication and problem-solving skills can make a real difference to patients and their families.
In this role, you will:

  • Work collaboratively with HiTH and hospital teams to coordinate safe, seamless care.
  • Support patients and families through their transition from hospital to home.
  • Stay connected with the team through daily huddles, sharing updates and discussing patient needs.
  • Keep care moving by maintaining accurate documentation and following up referrals.

Why join us?
As an Intake Registered Nurse, you’ll use your clinical skills to help connect patients with the right care, in the right place, at the right time.

  • Grow your skills – build experience in triage, clinical decision-making and care coordination.
  • Make a difference – help patients receive quality care in the comfort of their own homes.
  • Be part of something innovative – contribute to a modern approach to acute care and hospital avoidance.
  • Work with a great team – collaborate with supportive nursing, medical and multidisciplinary teams.
  • Keep developing – access comprehensive orientation, education and ongoing professional development.

What we're looking for

  • Organised, adaptable and able to manage competing priorities.
  • Proactive, resilient and committed to patient-centred care.
  • Confident coordinating care and working with multiple stakeholders.

Experience in Hospital in the Home, community or acute care is highly regarded. Knowledge of HiTH referral processes, eligibility criteria and escalation pathways is desirable, but can be supported through orientation and on-the-job learning.
