About the Role
The SLHD Community Transition Care Program is a goal-orientated program designed to assist eligible hospital inpatients to return to living at home safely, by providing low intensity therapies and assistance with activities of daily living for up to 12 weeks. 

The position is a community role, covering the Sydney Local Health District geographical area.  TACP has its office base at Concord Hospital, but you may be asked to commence or finish work from other SLHD locations which include RPA, Canterbury and Balmain Hospitals. There may be opportunities for mobile work arrangements and community based work-related activities will require use of your own car.

Ideal Candidate

 

  • Equivalent work experience in an allied health or health related setting and/or successful completion of Certificate III or Certificate IV Allied Health Assistant or other qualification deemed equivalent by the employer.
  • Demonstrated experience working with older people and or people with a disability
  • Current NSW driver's licence (P1 and P2 acceptable).
