Remuneration: $139,661.16 - $149,538.56 p.a. + Superannuation + Salary Packaging

We are excited to be advertising for a Nurse Practitioner experienced in aged and chronic care diseases to join our Aged Care Services team. In this role, you will provide expert clinical care to older adults with complex and chronic conditions, including dementia, cognitive impairments, sleep disorders, depression, and more. This role is key in improving health outcomes for older adults and promoting continuous service improvement in the community.


As a Nurse Practitioner, you will:
  • Provide expert clinical care focused on early diagnosis, pre-emption, and reducing hospitalisations for older adults
  • Manage complex conditions like dementia, cognitive impairment, sleep disorders, depression, anxiety, weight loss, and falls, through both home and clinic-based services.
  • Offering advice and care to prevent and manage complex health issues
  • Provide home and clinic-based services, working with a multidisciplinary team to achieve the best outcomes for patients
  • Collaborate with Aboriginal medical services to implement ambulatory care treatment for Aboriginal communities facing aged chronic health issues
  • Promoting best practices and early intervention strategies for aged care patients

Our ideal candidate:
  • Is an expert in aged care and chronic disease management 
  • Can work autonomously, and part of a multidisciplinary team to best cater for our diverse caseload of patients with varying needs
  • Has strong verbal and written communication skills to liaise with patients, families, and a variety of health professionals
  • Is committed to improving community health services and implementing innovative care models 


Essential requirements:
  • Current authority to practice as a Registered Nurse with AHPRA and current endorsement as a Nurse Practitioner or, if transitional, evidence of substantial progress towards endorsement.
  • The equivalent of three (3) years' full-time experience in an advanced practice nursing role within the area of intended practice, within the past six (6) years from the date when the completed application seeking endorsement as a Nurse Practitioner (Masters of Nursing-NP) is received by the Board.
  • Eligibility to drive in NSW.
