The primary purpose of this role is to provide comprehensive assessment and management with a multidisciplinary Aged Care team to ensure the provision of optimal and timely care to chronic and complex older patients. The Advanced Trainee will be rotating in Acute Geriatric Medicine, Sub-Acute Geriatric Medicine, Geriatric Rehabilitation, Psychogeriatric Medicine and the community including Orthogeriatrics and Care of Older patients in Surgery (COPS). The population group would be patients over the age of 70 or over 50 years for Aboriginal and Torres Strait Islander with chronic and complex issues.
The Advanced Trainee is required to move between services and geographical locations to deliver patient care including rotation within Nepean Hospital, Springwood Hospital and the community.
What you'll be doing
- Comprehensive assessment and management of Aged Care patients in the Acute Geriatric Service, Geriatric Rehabilitation/GEM, subacute and surgical patients in perioperative care. >
- Initial assessment of patients which consists of admission into Geriatric Rehabilitation/GEM Ward.
- Develop comprehensive medical plans for all patients in the Acute Geriatric and Rehabilitation Service.
- Once daily medical rounds, daily huddle across acute Aged care patients E and one weekly case conferences (N2G).
- Attending family conferences as required.
- Six monthly rotations through:
- Acute Geriatrics
- Geriatric Rehabilitation/GEM/Consults
- Periperative Care – Orthogeriatrics/COPS
- Community – ACAT/VACS and clinics
- Participating in 24/7 rotating roster of days, evenings, nights and weekends.
Where you'll be working
- The Nepean Aged Health Service provides a comprehensive array of services that cover the continuum of care across acute subacute, Geriatric Outreach services, in community including residential aged care.
- 56 and 4 ACRAU/MAUacute beds and 16 geriatric rehabilitation beds and 6 GEM beds.
- Acute services include:
- ACRAU – Aged Care Rapid Assessment Unit (4 beds)
- A multidisciplinary ASET team
- Two Acute Aged Care Wards
- Orthogeriatric service
- COPS – Care of Older Person in Surgery
- Subacute services include:
- Specialty clinics (Cognitive Disorders, General, Comprehensive Falls and Fractures) and PrEPP
- Springwood 10-15 subacute beds
- Aged Care Assessment Team
- VACs program provides Geriatric Outreach service in community and in RACFs via Single Point of Contact
- Innovative telehealth pilots with RACFs and outlying facilities

- MBBS or equivalent, currently registered with the Medical Board of Australia in an appropriate registration category.
- Completion of at least four postgraduate years and successful completion of the examination requirements of the Royal Australasian College of Physicians, or equivalent, and be eligible to undertake subspecialty training in Geriatric Medicine in Australia.
- Demonstrated commitment to training in Geriatric Medicine.
- Demonstrated clinical practice as a registered medical practitioner in the last 2 years in a setting appropriate to the needs of the position.
- Sufficient clinical experience in order to undertake the key accountabilities for this role.
- Demonstrated understanding of patient flow and activity based funding.
- This position is subject to an appropriate supervision level which can be supported by the health agency. If the incumbent’s registration conditions include supervision requirements, the level of supervision must be able to be supported by the health agency for this role.
- Valid Working with Children Check, cleared National Police Check.
- To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy, this includes any amendments made from time to time.
