Where you’ll be working:
An exciting opportunity exists for an Aboriginal Health Practitioner/Worker to join the Integrated Chronic Care for Aboriginal People Program, based at Inverell Community Health.
In this role, you'll work in partnership with a Clinical Nurse Consultant and a supportive multidisciplinary team to provide culturally safe, holistic care for Aboriginal people living with chronic and complex health conditions, including diabetes, cardiovascular disease, and respiratory illness. About you
Experience working with Aboriginal people living with chronic disease and providing health education and support will be highly regarded.
