About the role
Join the Integrated Chronic Care for Aboriginal People (ICCAP) program as an important role in improving health outcomes for Aboriginal people and communities across the New England region. Based in Inverell, this position will see you travel to outreach clinics in Glen Innes, Tenterfield, Guyra, Armidale, Uralla, Ashford and Tingha - delivering culturally safe, informed and community-focused care alongside a supportive multidisciplinary team.
Working closely with the Clinical Nurse Consultant (CNC), you’ll provide practical support and education to Aboriginal people living with chronic health conditions, along with helping mob understand and manage their health closer to home. A key part of the role includes point-of-care testing within community settings, as well as supporting care coordination and follow-up.
You’ll regularly collaborate with visiting specialists, including Dr Oakley, an Aboriginal physician from John Hunter, during clinics scheduled in Glen Innes and Inverell. You’ll contribute to specialist cardiology outreach clinics held throughout the year, working alongside cardiologists, echo technicians, Registered Nurses and fellow Aboriginal Health Workers & Practitioners in a highly respected and collaborative service for Aboriginal patients.
As part of the broader ICCAP network, you’ll be connected to a dedicated regional team who meet regularly to share knowledge, experiences and provide cultural and professional support to strengthen service delivery across communities. You’ll work alongside two other Aboriginal Health Practitioners/Workers locally, with ongoing supervision, mentoring and professional support available.
This role offers a balance of teamwork and autonomy, making it ideal for someone who enjoys building strong community relationships and engagement, working independently when needed, and using their organisational and technology skills to support culturally responsive quality care outcomes.

Requirements:
Hold or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification and/or Hold registration with Australian Health Practitioner Regulation Agency as an Aboriginal Health Practitioner
Additional Information:
- To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.
