What you'll be doing
As the Aboriginal Chronic Care Nurse, you’ll play a pivotal role in delivering culturally appropriate nursing services to Aboriginal clients with chronic and complex health needs. You’ll lead care coordination, advocate for community health priorities, and foster trust through respectful engagement.
Provide clinical leadership and mentorship within Aboriginal health services
Coordinate care and share information to support chronic disease management
Advocate for culturally safe and equitable health service delivery
Build strong relationships with Aboriginal families, carers, and community partners
Lead quality improvement initiatives and contribute to clinical practice development

Essential Criteria:

Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA) with recent postgraduate clinical nursing experience three (3) years full time equivalent post registration experience.
Selection Criteria:
This is an identified Aboriginal Position. Applicants must be of Aboriginal and / or Torres Strait Islander descent. An applicant’s race is a genuine occupational qualification and exemption is claimed under Section 14d of the Anti-Discrimination Act 1977, NSW.
Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA) with recent postgraduate clinical nursing experience three (3) years full time equivalent post registration experience.
Post registration graduate qualifications relevant to performing the role of Clinical Specialist Grade 2 for Aboriginal Chronic Care e.g. Advanced Nursing and/ Aged Nursing, Critical Care Nursing and /or Diabetes Management, and /or Renal Nursing or equivalent.
Demonstrated skills and knowledge in case management, including care coordination and education on self-management for Aboriginal people living with complex and chronic conditions, using culturally appropriate and advanced clinical skills in patient/client Assessment and care planning.
Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research and best practice Demonstrated ability to communicate effectively with Aboriginal clients, carers and a diverse range of health care providers including those in government, non-government and community organisations.
Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based services.
Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training.
Current Drivers Licence valid within Australia and willingness to use for work purposes.
