About the Role

We are looking for dedicated and experienced Patient Flow and Access Discharge Concierge Coordinators to join our team at Wyong Hospital. This pivotal role supports the achievement of key performance targets aligned with Ministry of Health (MOH) funding requirements.

Positions are based at Wyong Hospital and operate between 7:00am and 10:30pm, across a 7-day rotating roster covering morning and afternoon shifts. Flexible shift options will be considered.

As a Patient Flow and Access Clinical Nurse Coordinator, you will work in close collaboration with the Patient Flow Manager (PFM) and After Hours Nurse Manager (AHNM) to identify, monitor, and escalate any delays impacting the patient journey. You’ll provide vital support to Nurse Unit Managers by embedding best-practice discharge processes, assisting with discharge planning, generating and sharing patient flow reports.

This role is central to coordinating specialised care, strengthening communication, and enhancing the overall experience for patients and carers. Working alongside multidisciplinary clinical teams, you will help lead improvements in discharge practices to support efficient hospital operations and ensure high-quality, patient-centred care.

About You

  • You have current registration with AHPRA as a Registered Nurse (Division 1) with relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation. 
  • You have proven ability to analyse and interpret data related to patient flow and hospital access targets.
  • You have strong commitment to quality improvement, utilising evidence-based practices to assess care and drive change.
  • You have expertise in applying advanced clinical knowledge and judgment to deliver complex care and make informed decisions.
