Join Our Compassionate and Skilled Social Work Team at St George Hospital. We are seeking a level 1 / 2 Social Worker to join our dedicated team in a permanent full-time role within the inpatient General Medicine and Aged Care team at St George Hospital.

The level 1 / 2 Social Worker will work alongside a level 3 senior and be part of the Aged Care Team and will be the social work clinical lead in the General Medicine Unit. The patients that require admission to the General Medicine unit often have complex needs and need extensive case management and social work intervention. The issues faced by patients include issues of Aging, Dementia with Behavioural and Psychological Symptoms of Dementia (BPSD), Disability requiring National Disability Insurance Scheme (NDIS) support, drug and alcohol, mental health, and end of life. If you are looking for a work area which offers clinical challenges and variety of work and can bring established expertise in these clinical areas this position may be for you. 

Key responsibilities include: 
 
  • Previous experience working in Aged Care and General Medicine and/or knowledge of the issues of aging and complexities of healthcare.
  • Experience providing psychosocial assessment intervention for persons with complex care needs and families.
  • Supporting complex case management, discharge planning and grief and bereavement care.
  • Understanding of NCAT and NDIS processes and applications. • Collaborating closely with medical, nursing and allied health. We are looking for a Social Worker who brings sound clinical skills and emotional intelligence, who is passionate about advocating for vulnerable patients and supporting families through challenging health transitions. Selection Criteria
 

  1. Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award providing eligibility for membership of the Australian Association of Social Workers.
  2. Demonstrated core competency skills in psychosocial assessments, individual and family casework, crisis intervention, counselling, advocacy and discharge planning.
  3. A good knowledge of community resources and experience liaising with community organisations.
  4. Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients to enhance service delivery.
  5. Ability to work as part of a multi-disciplinary team and to exercise independent professional judgement on routine matters, commensurate with years of experience
  6. Computer skills and the capacity to use health information systems.
  7. Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
  8. Current drivers licence (or a willingness to travel in accordance with the demands of the position) and the ability to participate in the After Hours Crisis On-Call service, and work across a seven day roster (including weekends).
