About You
SWSLHD is seeking an enthusiastic Level 1 /2 Social Worker to join the Fairfield Transitional Aged Care Program (TACP) team. This community-based team serves to transition aged care clients safely from hospital back into the community by providing slow stream, goal based, therapy focused care.

As a TACP social worker you will be hardworking, positive, and responsive.  You will have strong skills in psychosocial assessment, linking clients to services and supports, and provide a range of social work interventions relevant to a community aged care caseload. Experience working in aged care and/ or rehabilitation is a benefit.  You will have strong communication, clinical judgment and problem solving skills.


About the Opportunity
This is an excellent opportunity for a social worker with a desire to work in a community setting who is dedicated to achieving positive outcomes for aged care clients.

TACP is a unique program focused on the reablement of older adults living in the community. Working with clients, their carer’s, the TACP multidisciplinary team and other relevant stakeholders, you will be contributing to holistic care that ensures better health outcomes and quality of life over a time-limited period post-discharge from hospital.  

 

What You'll be Doing
To provide Social Work services to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

  1. A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
  2. Demonstrated experience in a health related field and ability to provide a range of Social Work interventions
  3. Demonstrated experience prioritising work and managing stressful situations
  4. Proven ability to work independently and collaboratively within a multi-disciplinary team.
  5. Demonstrated high level communication skills (written and verbal) and computer skills
  6. Evidence of commitment to clinical supervision and professional learning
  7. Demonstrated understanding of, and commitment to quality processes, and the application of evidenced based practice within social work.
  8. Must possess a valid Australian Drivers Licence (excluding learner driver's licences). If not already in possession, a NSW Drivers Licence must be obtained within 3 months of appointment.
