Take the lead in shaping safe, efficient and patient‑centred care by driving excellence in patient flow, care coordination and multidisciplinary collaboration within General & Acute Medicine.
- Provide senior clinical leadership to coordinate referrals and ensure timely, seamless patient transitions across inpatient care areas.
- Drive discharge excellence by leading the Discharge by Midday and Next Day Discharge programs, optimising patient flow and bed capacity.
- Champion the Discharge Patient Flow Concierge Program to enhance patient, family and carer experience.
- Deliver advanced clinical guidance to support effective decision‑making and improve patient outcomes throughout the hospital journey.
- Strengthen operational performance through active participation in Electronic Journey Board Rounds and multidisciplinary team meetings, fostering clear communication across nursing, medical, allied health and support services.

- Proven leadership capability, with the confidence to coordinate care, influence clinical decision‑making and support teams.
- Highly organised and adaptable, with the ability to manage competing priorities while maintaining a patient‑centred focus.

- Describe your experience providing complex nursing care within an acute or general medicine setting. In your response, outline how you apply advanced clinical judgement, critical thinking and evidence‑based practice to deliver safe, compassionate and culturally responsive care.
Provide an example of how you have effectively communicated and collaborated with patients, carers and the multidisciplinary team to coordinate complex care. Include how you managed challenges such as patient flow, discharge planning, conflict resolution, or contributions to quality improvement, risk management or workplace safety.
