An exciting opportunity exists for a permanent part-time level 1 / 2 social worker to join the SOS team at Southcare. The SOS provides short term acute, and sub-acute clinical interventions for a maximum of six weeks. The aim of the SOS is to provide safe and effective interventions to allow clients at risk of presenting to the Emergency Department (ED)/ Hospital to remain at home. The SOS MDT works together in collaboration with GP’s, services and other community teams to ensure clients with health conditions and disabilities can remain safely at home. This SOS MDT is a supportive and inclusive team that includes: Nursing, Physiotherapy, Occupational Therapy, Social Work and Geriatrician’s.

The position is funded 29 hours per week however hours and/or days can be discussed for the successful applicant.

Located on the Sutherland Hospital campus; only a short distance from public transport, Miranda Westfield and Cronulla beach.

What you'll be doing

The Level 1/2 Social Worker is an integral and respected member of the MDT that makes up SOS team.

In this community-based role, you will support older adults and younger people with disability who are at risk of hospital admission, delivering high-quality care to help them remain safely at home.

Working as part of a supportive team, you will provide assessment, case management, advocacy, and coordination for clients with complex needs, while contributing your expertise to a respectful and inclusive team environment.

Selection Criteria:
1. Unrestricted NSW drivers licence.
2. Bachelor of Social Work and eligibility for membership of Australian Association of Social Workers (AASW)
3. Demonstrated understanding of aged care issues and younger clients with severe and chronic disability in a community setting.
4. Demonstrated organisational skills and ability to manage a demanding caseload.
5. Demonstrated high level communication, report writing skills and computer skills.
6. Demonstrated ability to work independently and as part of a multidisciplinary team with minimal direct supervision.
7. Demonstrated commitment to ongoing quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning.
