Where you'll be working:

Plan, coordinate and deliver high quality nursing care to patients consistent with the Southcare Outreach Service (SOS) service objectives and South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

The Southcare Outreach Service (SOS) provides a multidisciplinary community team for the ageing population over 65 years of age residing in the Sutherland Shire. The SOS provides short term acute interventions (up to six weeks) by Nursing, Physiotherapy, Occupational Therapy, Geriatrician and Social Work to facilitate enhanced patient care and safe clinical outcomes. SOS staff will provide assessment, intervention and referral onto other required services in order to navigate and integrate the patient’s journey between tertiary and primary care.

The Southcare Outreach Service (SOS) provides a 7 day a week service: public holiday coverage, afternoon shifts (1030 – 1900 hours) and some weekend cover will be required in this role.

    Selection Criteria:

    1. Current unrestricted registration as a Registered Nurse with the Nursing & Midwifery Board of Australia and current NSW driver’s licence and willingness to use same as required for the position.
    2. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.
    3. Demonstrated ability to proactively engage with patients/clients to enhance service delivery and to guide others in providing service excellence.
    4. Demonstrated ability to work as part of a multi-disciplinary team and apply professional and ethical boundaries when dealing with complex situations.
    5. Basic computer skills and proven ability to use health information systems.
    6. Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Registered Nurse in applying these principles.
    7. Demonstrated recent post registration experience working in an aged care setting with the aged population.
    8. Demonstrated experience in providing nursing assessment and interventions to the aged care client
