Where you'll be working
Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care.

What you'll be doing
Plan, coordinate and deliver high quality Aged Care Assessments to aged care clients and younger adults with severe and chronic physical disabilities who are referred through My Aged Care consistent with South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

As a Registered Nurse in SESLHD, you will be an integral part of the interdisciplinary team in the provision of person-centred care; and leading, supporting and supervising other nurses and care roles. You will strive to ensure that the people who receive healthcare experience exceptional care that is person-centred, safe, high quality and compassionate.
As a Registered Nurse in SESLHD you will be recognised as a healthcare professional employing your clinical skills and critical thinking in delivering high quality patient care.

As a Registered Nurse in Prince of Wales Community Health, you will be encouraged to share your clinical expertise and further develop your practice with the professional development opportunities available.
Some of the key functions of the Registered Nurse is to practice in accordance with the Australian Nursing and Midwifery Boards Standards of Practice:
- Think critically and analyse nursing practice
- Engage in therapeutic and professional relationships
- Maintain the capability for practice
- Comprehensively conduct nursing assessments
- Develop a plan for nursing practice
- Provide safe, appropriate and responsive quality nursing practice
- Evaluate outcomes to inform nursing practice
Selection Criteria:
- Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife. Current unrestricted NSW drivers licence and willingness to travel in accordance with the demands of the position
- Demonstrated ability to proactively engage with clients to enhance service delivery and to guide others in providing service excellence
- Demonstrated high level written verbal communication skills, interpersonal skills and be able to identify key messages, issues and concerns when communicating with others.
- Ability to work as part of an interdisciplinary team and apply professional and ethical boundaries when dealing with complex situations.
- Intermediate computer skills and proven ability to use health information systems.
- Demonstrated ability to advocate for and implement person-centred practice.
- Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Clinical Assessor in applying these principles
