What you will be doing

The Child and Family Health Nurse at the Rockdale Children and Families Hub plays an essential role in providing high-quality care to children aged 0 to 7 years, particularly from priority populations. This role involves working in an integrated, place-based care model, collaborating closely with both government and non-government partners, including early learning services, schools, family workers, and health agencies. The nurse helps families navigate available services, offering follow-up and feedback to ensure continuous care.

A key aspect of the position is the provision of holistic care, where the nurse assesses, plans, and delivers tailored nursing interventions with the family based on their needs. This includes health promotion activities, physical and psychosocial assessments to identify strengths and challenges, ensuring early intervention and appropriate referrals. The nurse also plays an active role in promoting health within the community and works to ensure that all families receive the necessary support to achieve the best health outcomes for their children.

To effectively carry out these duties, the nurse needs to hold post-graduate qualifications in Child and Family Health. This ensures they are equipped to work within a primary health care framework. The nurse will adhere to national competency standards and guidelines from the Australian Nursing and Midwifery Council (ANMC), as well as NSW Health and SESLHD policies, to ensure that all care provided aligns with professional standards and achieves the best outcomes for the families.
