Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.
 
PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.
 

PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.

What you'll be doing

The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional care, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.

The Registered Nurse will provide a comprehensive Child and Family Health Nursing service, with home visits, clinic visits and group work to Aboriginal families with children aged 0-5 years, within the context of current policies and “best practice”.

The Registered Nurse is responsible for the provision of person-centred, evidence-based nursing care that promotes positive health outcomes, using a culturally sensitive, trauma informed approach. 

 The Nurse will assess, plan, implement and evaluate nursing care in collaboration with individuals and the broader multidisciplinary team. This will involve undertaking physical, emotional, and psychological assessments to identify family strengths and vulnerabilities, to enable early identification and interventions.
 
There may be a requirement for the Registered Nurse to work in universal and/or targeted Child and Family Health Nursing services.

Selection Criteria

  1. Current Registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and current NSW Driver’s Licence.
  2. Recognised postgraduate qualification in Child and Family Health Nursing with experience in the field of Child and Family Health Nursing and/or experience working with Aboriginal families.
  3. Demonstrated high level written and verbal communication and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
  4. Demonstrated ability to work in partnership with families within a primary healthcare setting.
  5. Demonstrated ability to work as part of a multi-disciplinary team and apply professional and ethical boundaries when dealing with complex situations.
  6. Demonstrated understanding of breastfeeding and lactation.
  7. Basic computer skills and proven ability to use health information systems.
  8. Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Registered Nurse in applying these principles.
