Sydney Children's Hospitals Network is the largest paediatric health care entity in Australia. We care for thousands of children each year in our hospitals and in their homes — with one purpose in mind — to help young people live their healthiest lives.
Integrated Care Department at SCHN is expanding. The team is fostering integration between individuals, processes and systems to enhance human experience and achieve better outcomes for children, young people and families. We are seeking a highly motivated results driven individual to provide clinical expertise, project management and change management expertise to achieve integration of clinical processes around the individual patient and family level; and at the broader service, organisational and system levels to improve human experience and enhance efficiencies in the system. The candidate will need to be flexible, proactive, action oriented and productive individual, who can collaborate with multidisciplinary teams to facilitate integration outcomes. The candidate will have a passion to contribute to integrating health system for children, young people and families. The individual will be a team player who can work independently and will possess skills in clinical redesign/improvement science, change management, facilitation, and excellent verbal and written communication skills. The individual will carry out a variety of functions which may be complex in nature and require judgment in selecting and applying established and developing principles, techniques and methods.
