The Kids Guided Personalised Service (KidsGPS) is the hospital service supporting children with complex medical conditions and their families coordinate their healthcare journey. The goal of our team is to make this as easy as possible for your child and your family by improving how all the services work together. The KidsGPS Care Coordinator will work closely with you to plan your child’s care and provide support as the main point of contact. Care coordinators work as health care guides, connecting the different services and empowering you to advocate for your child’s care. They will work with you to decide how long you may need the support for.
What you'll be doing
The Clinical Nurse Specialist Grade 2 (CNS2) is an extended role within the scope of a Registered Nurse/Midwife that provides safe, compassionate, person-centred care to patients and families of the Sydney Children’s Hospital Network
Acts as an appropriate and effective role model to promote culture and standards that reflect the NSW Health CORE values of Collaboration, Openness, Respect and Empowerment.
Provides senior nursing clinical leadership in the development of specialist clinical practice and service delivery within a nursing unit, team or service. Provides advanced skills and exercises extended autonomy of decision making while working closely with managers and other senior clinicians.
Exercises extended autonomy of decision making; exercises professional knowledge and judgement in providing complex care requiring advanced clinical skills.
Undertakes leadership in the development of nursing specialty clinical practice service delivery in the ward/service; or undertakes primary case management within a defined specialty involving both inpatient and community based services.
About You
We’re looking for someone who:
- Current Authority to Practice as a Registered Nurse with AHPRA and relevant post-registration qualifications and at least 3 years' experience working in the relevant clinical area of their post-registration qualification.
- Demonstrates evidence of advanced clinical practice in the specialty area. Demonstrates evidence of contribution to the development of clinical practice relevant to paediatrics.
