Nurse Practitioner / Transitional Nurse Practitioner – Palliative Aged Care Supportive Service (PACSS)
- Deliver advanced clinical assessment, care planning, and evidence-based interventions.
- Support coordinated, person-centred care aligned with NSW Health End of Life and Palliative Care Frameworks.
- Facilitate timely and appropriate transitions to end-of-life care.
- Promote advance care planning and optimizing symptom management.
- Providing support and coordinating of care for people approaching the end of life in RACFs.
- Provide high-level clinical expertise in advanced palliative care nursing.
- Lead and support innovative models of care to those patients and their families in their transition to end of life care.
- Collaborate with multidisciplinary teams and key stakeholders to ensure integrated service delivery.
- Support education, mentoring, and professional development of nursing, allied health staff, and carers.
- Strengthen clinical governance, quality improvement, and patient safety initiatives in line with NSW Health policies and standards.
- Endorsed Nurse Practitioner or Transitional Nurse Practitioner (or working toward endorsement).
- Extensive experience in palliative care, with demonstrated expertise in aged care settings.
- Strong leadership, communication, and stakeholder engagement skills.

Candidates will need to meet the following criteria:
- Registered Authorised NP with APHRA that has completed the Tertiary requirements including a Master’s Degree (Nurse Practitioner) OR Registered Nurse willing to enrol in a Tertiary Master’s Program for Nurse Practitioner.
- Minimum of 5,000 hours of advanced nursing/midwifery practice during the last six years appropriate to the specialty area (or working towards within the next 3 years).
- Ability to independently assess, investigate and implement appropriate treatment for patients within advanced nursing practice guidelines.
- Demonstrated clinical competency in advanced practice management in Supportive and Palliative Care and specific interest in aged care.
- Demonstrated ability to work autonomously and have effective communication and negotiation skills.
- Demonstrated ability to work independently within the wider multidisciplinary team.
- Current Driver’s Licence
