Nurse Practitioner / Transitional Nurse Practitioner – Palliative Aged Care Supportive Service (PACSS)

An exciting opportunity exists for a highly skilled Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) with advanced clinical expertise in Palliative Care and a strong interest in Aged Care to join the Palliative Aged Care Supportive Service (PACSS).

In this role, you will deliver advanced nursing assessment, diagnosis, and clinical care to patients and their families within a defined scope of practice, consistent with the NSW Health Nurse Practitioner Framework and relevant professional standards. You will also provide clinical leadership, contribute to strategic service development, and support governance processes to strengthen models of care for residents transitioning to end-of-life care.

About the Role

As the NP/TNP within PACSS, you will provide expert palliative care to older people residing in Residential Aged Care Facilities (RACFs) across the Ryde and Royal North Shore catchment.

Working collaboratively with the NSLHD Specialist Palliative Care Service, the Aged Care Rapid Response Team (ARRT), General Practitioners, and HammondCare Palliative Care, you will:

  • Deliver advanced clinical assessment, care planning, and evidence-based interventions
  • Support coordinated, person-centred care aligned with NSW Health End of Life and Palliative Care Frameworks
  • Facilitate timely and appropriate transitions to end-of-life care
  • Promote advance care planning and optimizing symptom management
  • Providing support and coordinating of care for people approaching the end of life in RACFs

Key Responsibilities

  • Provide high-level clinical expertise in advanced palliative care nursing
  • Lead and support innovative models of care to those patients and their families in their transition to end of life care.
  • Collaborate with multidisciplinary teams and key stakeholders to ensure integrated service delivery
  • Support education, mentoring, and professional development of nursing, allied health staff, and carers
  • Strengthen clinical governance, quality improvement, and patient safety initiatives in line with NSW Health policies and standards

  • Endorsed Nurse Practitioner or Transitional Nurse Practitioner (or working toward endorsement)
  • Extensive experience in palliative care, with demonstrated expertise in aged care settings
  • Strong leadership, communication, and stakeholder engagement skills

Candidates will need to meet the following criteria:

  1. Transitional (Trainee) Nurse Practitioners must hold a current AHPRA registration as a Registered Nurse with endorsement to practice as a Nurse Practitioner or currently enrolled in an approved Nurse Practitioner Masters program working towards endorsement.
  2. Transitional (Trainee) Nurse Practitioners must have a minimum of 5,000 hours of advanced nursing/midwifery practice during the last six years appropriate to the speciality area (or working towards within the next 3 years).
  3. Demonstrated clinical competency in advanced practice management in Supportive and Palliative Care and specific interest in aged care.
  4. Demonstrated ability to work autonomously and have effective communication and negotiation skills.
  5. Demonstrated ability to work independently within the wider multidisciplinary team.
  6. Hold current Driver’s License.
