Where you'll be working
Based at Macquarie Hosp but working across the Northern Sydney LHD

What you'll be doing
Home Support Assessors provide basic home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).
Working with each client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including generating codes for services for referral to age care and non-aged care services, facilitates a referral and develops a Support Plan. The Home Support Assessor completes the IAT and matches and refers clients to appropriate service(s).  As the role’s purpose is to provide assessment services across the district, some flexibility regarding the location of the role may be required depending on client needs.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:

  1. Well-developed written, verbal and interpersonal communication skills.
  2. Demonstrated computer skills with capacity to learn and use electronic reporting systems.
  3. Demonstrated ability to work independently as well as effectively in a collaborative team environment.
  4. Demonstrated equivalent experience working with older people and/or Certificate III in Home Care or Aged Care or Allied Health Assistance.
