Based at Mona Vale Hospital, Hospital in the Home is an acute care service across the Northern Sydney Local Health District (NSLHD) caring for acute but stable patients in their home. We are looking for senior registered nurses with strong acute care clinical skills and strong communication skills to join our multidisciplinary team.

What you'll be doing
As an independent practitioner, the Registered Nurse (RN) will provide holistic care for acute patients in their home, escalating any concerns or deterioration and communicating to the multidisciplinary team. The RN must have excellent assessment skills and confidence to problem solve in unfamiliar environments. As a flexible RN, you will, at times, be asked to be team leader, hospital liaison, or work in one of our clinics across the service.

Candidates will need to meet the following criteria:

 

  1. Registered Nurse –Division 1 with Australian Health Practitioner Regulation Agency (AHPRA); and Current NSW Drivers Licence.
  2. Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.
  3. Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.
