Clinical Nurse Specialist, Care Coordination Team

- Division 1 Registered Nurse with post registration qualification relevant to speciality and at least 3 years experience working in the clinical area of their post graduate qualification or such other qualifications or relevant equivalent experience.
- Evidence of and commitment to patient/customer focused care, clinical governance and quality improvement
- Evidence of advanced level of knowledge and skill related to management of speciality area
- Demonstrated knowledge in initiating and implementing changes to improve clinical service delivery and outcomes, models of care, evidence based practice and practice development.
- Demonstrated excellence in written and verbal communication skills and computer literacy in Microsoft Applications.
- Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion.
