Aged Care Rapid Response Team Clinical Nurse Specialist

1. Current Registered Nurse, registered to practice with AHPRA. Hold relevant post graduate qualifications with at least 3 years’ recent experience in the speciality of Aged Care and /or Community Nursing.
2. Demonstrated well developed written, verbal and interpersonal communication skills, including computer skills, with demonstrated experience in or willingness to learn and use electronic clinical/medical/patient information and reporting systems.
3. Demonstrated ability to work effectively and problem solve both independently and as a member of the multidisciplinary team.
4. Demonstrated knowledge of and/or experience in initiating and implementing changes to improve clinical service delivery and outcomes through evidence based practice and practice development.
5. Demonstrated ability to undertake quality improvement activities and promote an environment that encourages the application of these principles.
6. Current unencumbered NSW Driver’s Licence.
