About the Opportunity

The Community Nursing Assisted Transfer team supports vulnerable or at risk patients in getting home safely from hospital when other options are not available. This service operates 7 days a week between the hours of Mon - Fri - 8.00am - 7.30pm, Sat & Sun 8.00am - 4.30pm.

As an Enrolled Nurse, you will work alongside our Registered Nurses as part of the health care team and demonstrate competence in the provision of person-centred care

This position plays a key role in ensuring continuity of care and enhancing the overall patient experience. In this role you will:

  • Deliver direct, person-centered care that supports patients’ personal care, mobility, and communication needs.

  • Work under the direct or indirect supervision of the Registered Nurse (RN).

  • Engage in analytical thinking; use information and/or evidence; and skilfully and empathetically communicate with all involved in the provision of care.

  • Collaborate and consult with health care recipients, their families and community as well as RNs and other health professionals.

About You

We are looking for someone who has:

  • Must have current driver licence. We will not be able to consider applicants without a licence to drive a car in NSW.

  • Current Authority to Practice as an Enrolled Nurse (Division 2) with the Australian Health Practitioner Regulation Agency (AHPRA) with a Nursing and Midwifery Board of Australia approved qualification for administration of medication (including intravenous medication administration).

  • Nursing knowledge and skills relevant to the position speciality.

  • Demonstrated effective clinical communication through the use of information technology and written, verbal and interpersonal skills.

  • Understanding of team nursing & demonstrated ability to work effectively as part of the multi-disciplinary health care team.

  • Demonstrated ability to effectively prioritise and organise own workload.
