About the role

 

Lismore Base Hospital is seeking a dedicated Nurse Manager (Grade 2) – Transfer of Care to join the team.

 

In this role, you will lead and coordinate patient flow across the facility, overseeing bed management and discharge planning to ensure efficient, safe, and high-quality care. As part of the management team, you will support clinical staff in the planning, delivery, and evaluation of integrated patient care, while ensuring services are well-managed and responsive to individual patient needs. You will also play a key role in strengthening connections with external services to support seamless transitions of care.

 

This position offers the opportunity to contribute to a health system that values strong clinician leadership.
