Nepean Hospital is seeking an Enrolled Nurse Clinical Care Coordinator to play a key role in coordinating daily clinical activities, supporting patient flow, and ensuring person-centred care within the Aged Care/Rehabilitation unit. You will work closely with the Multidisciplinary team—including Registered Nurses, Allied Health, Medical Officers, and Support Staff to facilitate smooth transitions of care and promote safe and effective recovery. This is a fantastic opportunity for an experienced Enrolled Nurse who is passionate about supporting patient journeys, coordinating care, and contributing to high-quality outcomes for older adults and those undergoing rehabilitation.

You will be confident in communication, organisation, and prioritising care needs, able to work collaboratively within a multidisciplinary team, passionate about advocating for older adults and supporting their independence and motivated to contribute to continuous improvement and positive ward culture.

What you will bring to the role: 

  1. Current Registration to practice as an Endorsed Enrolled Nurse with the Australian Health Practitioner Regulation Authority (AHPRA)
  2. Ability to work independently and within the multidisciplinary team
  3. Understanding of Aged care and Rehabilitation principles including appropriate identification and referral protocols
  4. Demonstrated experience in a clinic setting including reception duties, phone enquiries, appointment scheduling and data entry
  5. To maintain a high level of personal professional and clinical performance by active participation in appropriate continuing education and professional development.
  6. Ability to work with the acute care multi-disciplinary services within both specialities
  7. Demonstrated ability to prioritise and manage own workload and meet benchmark responsibilities times.
  8. Familiar with Health IT platforms
