Where you'll be working:
This role is part of the Social Workers in Medicine Team within the Social Work Department at John Hunter Hospital (JHH), a large tertiary referral hospital providing specialised care to patients across the region. Your primary focus will be working with patients experiencing Stroke and Neurological conditions, as well as a small number of Spinal Cord Injury Rehabilitation inpatients.
You will work closely within the Clinical Stroke Multi-Disciplinary Team and collaborate with ward teams to provide comprehensive support to patients and their carers.
About the role:
As a Senior Social Worker, you will:
- Provide specialist social work services to patients experiencing stroke and stroke-like symptoms, as well as their carers/support networks.
- Conduct assessments, set patient-centred goals, and develop safe discharge plans with appropriate referrals to community-based services.
- Work in a fast-paced environment, responding efficiently to referrals and adapting to changing priorities.
- Use multiple technology-based hospital systems to support effective patient care.
- Mentor early-career social workers and social work students, supporting their professional growth.
- Collaborate within a multi-disciplinary team, working alongside health professionals to achieve the best outcomes for patients.
About you:
- Must hold as a minimum a Bachelor or Master’s degree in social work which provides eligibility for membership of the Australian Association of Social Workers.
- Experienced in providing social work services in a hospital or health-related setting.
- Highly organised and adaptable, able to work efficiently in a busy environment.
- Confident using technology-based hospital systems to manage patient care.
- A team player, keen to collaborate with multi-disciplinary teams and other social workers.
- Passionate about supporting patients, carers, and early-career social workers.

- Supervision and guidance from the Service Manager, Team Leader, and senior Social Workers.
