Where you'll be working:

Join the Social Work Team at Belmont Hospital, a supportive and collaborative environment where you'll provide vital social work interventions to inpatients and their families. Working alongside Allied Health, Nursing, and Medical teams, you’ll play a key role in discharge planning, psychosocial support, and linking patients to essential services.



About the role:

As a Social Worker at Belmont Hospital, you will:

  • Provide comprehensive social work assessments to inpatients in Medical and/or Surgical wards.
  • Work as part of a multi-disciplinary team, ensuring patients receive holistic and person-centred care.
  • Support safe discharge planning, including referrals to community services, NCAT applications, and NDIS support.
  • Assist patients and families with decision-making and goal setting, including guidance on Residential Aged Care options.
  • Advocate for patients and connect them to appropriate health and community resources.
  • Manage a diverse and dynamic caseload, requiring strong organisation, flexibility, and problem-solving skills.



About you:

  • A reflective and adaptable clinician, willing to develop your assessment and planning skills.
  • Strong ability to prioritise tasks and manage a fast-paced hospital workload.
  • Effective communicator, able to work well within a team and build relationships with patients, families, and other professionals.
  • Passionate about supporting vulnerable individuals, ensuring they receive the care and resources they need.



Support & Training:

  • Team Leader (Level 4) and Service Manager (Level 7) support.
  • Supervision from experienced Social Workers (Level 3 & Level 1/2 colleagues).
  • An individualised development plan tailored to your training needs.
