About the role:
This Occupational Therapist position is with the Transitional Aged Care Program, which is a short term (up to 12 weeks) Commonwealth funded aged care service for older people leaving hospital who would usually be eligible for ongoing care in a residential aged care facility but who have the potential to improve their physical, cognitive, or emotional function and wish to try and remain in their home. Our team covers the Lower Mid North Coast region and is based in Taree as part of Aged Care Services.
The community based Transitional Aged Care Program (TACP) serves to transition aged care clients safely from hospital back into the community. The TACP Occupational Therapist, in conjunction with the multidisciplinary TACP team, will work with clients to achieve their goals and ensure safe discharge from the program. You will be required to conduct visits in the patient’s home, working towards the patient’s goals.

The successful applicant will be a personable, dedicated professional who has a strong commitment to providing excellent patient focused care. Your outstanding interpersonal skills will be key to working and communicating with older adults in their home, by providing support, therapy and guidance to improve their independence and achieving their goals in accordance with the TACP Guidelines. The services provided will be dependent on clients’ needs and will vary accordingly. In order to work in the TACP team, attributes such as flexibility, the desire to work together as a team and great communication skills, both verbal and written, are required. As a Level 3 Occupational Therapist you will also be required to provide support and mentoring to junior Allied Health staff.
You will be well supported by the Service Manager of the Aged Care Services team. The team consists of NUM, Allied Health, TACP, ACAT, Psychogeriatrics, ASET, AARCs & Discharge Planning roles. Online education and the opportunity to attend external training sessions is available in order for you to fulfil your role.
