About the role

The Palliative Care Clinical Nurse Specialist Grade 2 (CNS2) works with members of the palliative care nursing team across the inpatient and community settings to provide a consultative and liaison focussed service aimed at supporting GPs, Community Nurses and the Residential Aged Care In-Reach Team in the assessment, care and management of patients with life limiting illnesses.
 
The aim is to facilitate a coordinated, high quality palliative care service to the local community, with a case coordination model that involves generalist community nurses in providing direct clinical care. The aim is to use a multidisciplinary approach to care, with the end goal of improving quality of life for those patients and their carers.
 
The CNS2 coordinates and delivers high quality nursing care to patients/clients in accordance with the Nursing and Midwifery Board of Australia’s National Standards of Practice for Registered Nurses, criteria for CNS2 as outlined in the Public Hospital Nurses and Midwives (State) Award 2017 and the Policies and Procedures of the Ministry of Health (MoH) and Northern NSW Local Health District (NNSWLHD).
