The Illawarra Transitional Aged Care Service is seeking two motivated Registered Nurses to join the team. We currently have positions available for 1 Full Time Permanent position and 1 Temporary Part Time 28 hours per week until 30 June 2025 with the possibility of an extension subject to funding.

What you'll be doing

Transition Care supports older people to return home from hospital, to build condition and capability. You will assess, plan, implement and evaluate nursing care in collaboration with individuals and the multidisciplinary team. You will need to understand complex health conditions in the older population and have knowledge of the Aged Care Principles.  Experience in Aged Care, Acute/Sub-Acute, Rehabilitation or Community settings would be transferable to this position. The office is currently located at Port Kembla Hospital with a fleet vehicle provided. 

All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (2 doses) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

 

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.

Selection Criteria 

  1. Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse (Division 1).
  2. A minimum of one (1) year full-time postgraduate employment (or 1982 hours part time/casual) within Australia.
  3. Recent clinical experience in a health care setting.
  4. Demonstrated understanding of evidence based practice and critical thinking.
  5. Demonstrated commitment to person centred care and NSW Health Core Values.
  6. Demonstrated high level verbal and written communication skills.
  7. Demonstrated ability to work as part of a multi-disciplinary team.
  8. Computer literacy and understanding of relevant health related information systems.
