ITACS provides a time-limited, goal-oriented service for older people who would benefit from additional low intensity therapy in their own homes, following discharge from hospital, as well as assisting with the decision-making process around long term care planning.
As the Senior Social Worker you will provide clinical leadership within social work service provision including optimisation of client care delivery, as well as supporting the mentoring and development of staff. The social worker will maintain a case load including case management responsibilities for a number of clients from admission to discharge. The role includes comprehensive psycho-social assessments, care planning, risk assessment, education, advocacy, counselling, liaison with community agencies and relevant parties including medical teams. Support often extends to client’s families and/or carers. Working in the community setting requires a level of autonomy in problem-solving without direct clinical supervision when completing in-home assessments and interventions.
The successful candidate will work as part of a multidisciplinary team, adopting a collaborative approach to delivering low-intensity, home-based therapy programs. These programs support frail and often socially complex older adults who are ready to be discharged home but require additional assistance and support to do so safely.
