Provide a high quality social work service to patients, clients, consumers and their family/ carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position provides the above social work service to the Illawarra Transitional Aged Care Service (ITACS).

ITACS provides a time-limited, goal orientated service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision making process around long term care service supports.

The social worker will also maintain social work and case management responsibility for a number of cases until discharge. Duties include early psycho-social assessments, care planning, education advocacy for the needs of clients, families and/ or carers, liaise with community agencies, the client's General Practitioner, counselling and carer support.

The position functions as part of an inter-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care, but are unable to return home without additional care and support.
