As part of the Community Health team, you’ll work with a supportive multidisciplinary network that includes community nurses, a Nurse Unit Manager, Dietitian, and a Diabetes Nurse Practitioner—all committed to improving the lives of people with diabetes across the region.
About the role:
As an experienced Diabetes Educator, you’ll deliver high-quality, individualised education and support to people living with various types of diabetes in both inpatient and outpatient settings. Your work will include assessments for type 2 and gestational diabetes, insulin initiation, and providing support to newly diagnosed patients. This role operates within weekday office hours, though travel to nearby locations is expected.
You’ll be part of a cohesive team and will have the opportunity to celebrate patient progress, support behaviour change, and work closely with other health professionals to achieve strong outcomes in chronic disease management.
About you:
You are a compassionate, knowledgeable Registered Nurse with a passion for improving the lives of people with diabetes. You are adaptable, self-motivated, and capable of working both independently and collaboratively.
You will bring:
- A Graduate Certificate in Diabetes Education and Management (or willingness to work towards it)
- Demonstrated experience in diabetes care, including insulin management and client education
- Familiarity with diabetes technologies (e.g. CGMs and insulin pumps)
- Excellent communication and documentation skills
- Emotional resilience and a positive, patient-focused attitude
Requirements:
- Current Authority to Practice as a Registered Nurse with AHPRA
- Graduate Certificate in Diabetes Education and Management (or working towards)
- Eligible to drive in NSW and willing to travel between sites
