About the role

You’ll start your day planning your schedule before heading out into the community, delivering safe, person-centred care directly in clients’ homes and local settings.
In this role, you’ll:

  • Provide wound care, medication administration, health assessments, chronic disease support, and health coaching.\
  • Use your clinical judgment to identify changes, escalate concerns, and adapt care plans.
  • Build supportive relationships with clients, families, GPs, and other health professionals.
  • Maintain accurate documentation to ensure seamless continuity of care.

You’ll be supported with a structured orientation, ongoing guidance from the Nurse Unit Manager, and a collaborative multidisciplinary team. Exposure to a wide range of clinical presentations makes this role an excellent opportunity for professional growth in community and chronic disease nursing.

Who we're looking for
We’re seeking a motivated, adaptable Registered Nurse with current AHPRA authority to practice, who can balance independence with teamwork. 
You are someone who:

  • Communicates clearly and manages time effectively.

  • Thrives on building meaningful relationships with clients in their homes.

  • Has experience in wound management and/or relevant qualifications, with strong clinical skills in assessment and chronic disease support.

  • Approaches every interaction with compassion, professionalism, and a genuine commitment to person-centred care.

Requirements 

  • Current Authority to Practice as a Registered Nurse with AHPRA

  • Demonstrated experience in wound management and / or qualifications
