About the Role 

Canterbury Hospital is committed to providing high quality healthcare services to our local community, focused on patient and family-centred care.

The Community Mental Health / Older People’s Mental Health Team  model of care states: The “Community Older People’s Mental Health Team” (OPMH Team) is a specialty service of the community mental health system. 

OPMH Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination. 


Consumers under the care of OPMH Teams will either have:   

  1. Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months. 

  2. A period of treatment as recommended by evidence based practices. 
     

The clinician will practice in keeping with the standardised Sydney LHD position description for the profession in 
addition to this position description.

Ideal Candidate

  1. Must hold a Bachelor degree in Social Work or equivalent that provides eligibility for full membership of the Australian Association of Social Workers.
  2. Demonstrated competencies to provide contemporary mental health practice.
  3. Current NSW Driver’s Licence (P1 and P2 acceptable).
