Are you an experienced Social Worker looking for a meaningful role where you can truly impact lives? Join our close-knit team at Scone Community Health for 1 day per week. You’ll job-share with another experienced Social Worker and collaborate with a multidisciplinary team including allied health, medical, and nursing staff, while connecting with our Social Work teams and Aboriginal Health Liaison Officers based at Muswellbrook and Singleton.
Deliver Expert Social Work Care in Hospital and Community Settings
In this role, you will provide therapeutic support and Social Work interventions to patients, clients, carers, and families attending Scone Hospital and Community Health.
In the hospital setting, you will deliver short-term interventions as part of the multidisciplinary team, contributing to high-quality, patient-centred care. You will actively participate in discharge planning, consultations, and networking meetings, and provide guidance, education, and support to staff on complex Social Work matters.
In the community setting, you will provide individual or group therapeutic counselling and support, while working collaboratively with community service providers and external stakeholders to achieve optimal outcomes for clients and their families.
