About the role:
Are you passionate about making a meaningful difference in the lives of individuals with dementia and their families? In this rewarding role, you will play a key part in organising a Clinic for Dementia and Aged Care alongside a Rehabilitation Specialist while also providing vital support through home visits. This is a fantastic opportunity for a compassionate and self-motivated professional who thrives in a person- and family-centred care environment and enjoys working both independently and within a collaborative, multidisciplinary team.
We are looking for someone with excellent communication, assessment, and time management skills, a strong work ethic, and the ability to connect with people from diverse cultural backgrounds. In return, you’ll receive comprehensive support, including online training, guidance from an experienced team member, and the opportunity to participate in Dementia Partnership meetings. About you:
- You’re a compassionate and proactive clinician who connects easily with patients and families
- You’re confident in working autonomously and as part of a multidisciplinary team
- You possess strong time management, assessment, and communication skills
- You bring a genuine interest in dementia care and an eagerness to support carers and community teams
- You have the cultural awareness and empathy to support diverse populations
Requirements:
- Current registration with AHPRA as a Registered Nurse
Proven clinical experience in dementia, delirium, or aged care case management
