About the role:
The AARCS clinician provides specialised aged care services for older individuals in inpatient settings with complex care and health needs. The service offers a patient-centred approach, focusing on comprehensive assessment, care planning, and coordination to ensure the timely and appropriate discharge of older patients.
The role requires effective time management to prioritise workloads, ensuring the older person, their carer/family, and the healthcare facility's needs are met, including providing the first face-to-face AARCS intervention within one working day of referral. Clinicians advocate for older patients and their families while balancing conflicting priorities and must communicate effectively with families and service providers to achieve the best outcomes. They make critical decisions regarding clinical care, assessment, case management, and discharge planning, with support from the Deputy Director of Nursing.
