Where you’ll be working
The Acute to Aged Related Services (AARCS) team provides specialised care for older patients in acute inpatient settings who have complex care needs. This service is designed to deliver patient-centred, comprehensive assessment, care planning, and coordination to enable safe and timely discharges. Working within a collaborative, supportive culture, you’ll be part of a team that consistently advocates for older people and their families, ensuring their needs remain at the centre of care.



About the role
As a Clinical Nurse Specialist 2 in AARCS, you will provide advanced clinical care, leadership, and expertise in managing complex cases. A key part of your role will be delivering the first face-to-face assessment within one working day of referral, then working with families, carers, and service providers to develop safe discharge plans. You’ll balance competing priorities, manage patients exhibiting challenging behaviours, and advocate for older patients while guiding and supporting your colleagues. Each day will bring opportunities to demonstrate clinical leadership, apply advanced assessment and case management skills, and contribute to improving outcomes for older people.



About you

  • Advanced clinical knowledge and experience in caring for older patients.
  • Skilled in managing patients presenting with challenging behaviours.
  • Ability to prioritise workloads effectively and meet referral response timeframes.
  • Strong advocacy skills, ensuring older people and their carers remain central to care decisions.
  • Excellent communication skills to engage families and service providers with differing perspectives.
  • Confidence in making clinical decisions around assessment, care planning, case management, and discharge.



Requirements

  • Current registration as a Registered Nurse with AHPRA.
  • Relevant post-registration qualifications and experience appropriate to the role of CNS2.
