What you'll be doing
- The Physiotherapist is a key member of the community-based, multidisciplinary Aged Care Outreach Service (ACOS) team, providing specialised care to residents of Residential Aged Care Facilities (RACFs) who are admitted to hospital within the Far West Local Health District (FWLHD).
- The Physiotherapist is responsible for delivering evidence-based assessment, rehabilitation, and treatment to support recovery, facilitate a safe and timely return to the RACF, and continue care until functional goals and baseline abilities are regained.
- The physiotherapist may be required at times to provide assessments and treatment to patients within the varied caseloads at the Broken Hill Hospital and Health Service, and outreach locations. This includes Adult and/or paediatric caseload areas which may include but not limited to inpatient acute wards, sub-acute rehabilitation, outpatients and outreach services.

How to Apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible by selecting APPLY NOW below:
- Tertiary qualification in Physiotherapy and current registration with the Australian Health Practitioner Regulation Agency (AHPRA), with eligibility for professional membership. At least 3 years post graduate clinical experience in a relevant field will be required to be considered for employment at level 3.
- Demonstrated clinical experience in rehabilitation and/or aged care physiotherapy, including the assessment and management of older adults with complex conditions such as post-fracture, stroke, and medical deconditioning
- Knowledge of discharge planning processes and experience in facilitating transitions of care from hospital to residential aged care or community-based settings.
- Ability to deliver physiotherapy services both in hospital and RACF environments, including the use of outreach or telehealth where appropriate, and to continue rehabilitation support post-discharge.
- Strong communication, documentation, and interpersonal skills, with the ability to work effectively with multidisciplinary teams, RACF staff, GPs, private providers and family/carers.
- Experience in education and support of care staff, including the ability to build capacity within aged care teams to implement mobility and rehabilitation strategies.
- Current unrestricted NSW Driver’s Licence, and willingness to travel across the Far West LHD to deliver care.
- For level 3 applicants only: Demonstrated leadership and supervision experience, including the ability to support junior staff and contribute to strategic planning and service improvement initiatives.
