About the Opportunity

As an Inpatient Social Worker (Level 1/2), you will work as part of a collaborative multidisciplinary team, delivering high-quality, trauma-informed, person-centred, and culturally responsive social work services. Every day presents an opportunity to make a genuine impact by supporting patients and their families through illness, crisis, recovery, and transition from hospital to community care.

This position provides compassionate, trauma-informed social work services that support patients and their families throughout their hospital journey and transition to community care. In this role you will:

  • Deliver trauma-informed, person-centred social work services, including psychosocial assessment, crisis intervention, counselling, advocacy, and care planning.
  • Support patients, families, and carers to navigate the emotional and practical impacts of illness while facilitating safe discharge and community-based care.
  • Collaborate with multidisciplinary teams and community partners to coordinate holistic, integrated care and achieve the best possible patient outcomes.
  • Contribute to continuous service improvement, professional development, and the after-hours Social Work on-call roster to provide timely support when required.

About You

We are looking for someone who has:

  • A tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
  • Demonstrated experience delivering psychosocial assessments, crisis intervention, counselling, advocacy, and care coordination in a clinical or healthcare setting.
  • Strong knowledge of trauma-informed, person-centred, and culturally responsive practice, with a commitment to evidence-based care.
  • Excellent communication and relationship-building skills, with the ability to collaborate effectively within multidisciplinary teams and with community partners.
  • A current Driver Licence, Working With Children Check (WWCC), and National Criminal Record Check (or the ability to obtain these prior to commencement).
