About the Role

  • Ongoing health monitoring of individuals and communities.
  • Health promotion and maintenance, early disease detection, and timely intervention.
  • Rehabilitation and palliative care, supporting optimal function or comfort.
  • Care planning and case management that empowers clients and supports families.
  • Developing responsive strategies to adapt to the changing health needs of the community.
  • You'll be part of a multidisciplinary team committed to promoting health equity and improving outcomes through client-centred, culturally safe, and socially responsive care.

About You
You have:

  • Holds current registration with AHPRA as a Registered Nurse.
  • Holds a current NSW drivers Licence and able to travel around the Central Coast.
  • Demonstrates a strong understanding of primary health care principles and community-based care.
  • Can assess, plan, deliver and evaluate care across various community health scenarios.
  • Holds recent nursing experience in Community Nursing or a demonstrated interest in Community Nursing.
