The Specialist Palliative Care Service is based on the Long Jetty Health Care campus, and requires a temporary full time or job share, dynamic and motivated Clinical Nurse Consultant Grade 2 to work across the Central Coast

 

About the Role

This community-based role is required to provide complex clinical support within Residential Aged Care Facilities (RACF) and to collaborate with RACF clinical care teams, the CCLHD Aged Care Response Team and the Palliative Care Liaison Nurses to undertake a project implementing Palliative Care Needs Rounding. Work hours are 0830 to 1700 Monday - Friday. The position is varied and interesting and requires monitoring of project progress in line with Ministry of Health CPCiAC requirements.

A solid individualised orientation program is provided for new Team members, where you will learn all aspects of the role. We are a small multi-disciplinary team that works collaboratively to support patients and families across the Central Coast.

About You

  •        The role requires a good knowledge of Microsoft Word, Excel, Outlook and eMR. Solid communication skills, relevant clinical experience and strong problem-solving abilities will contribute to the position's success.
  •        Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
  •        A current Australian driver's license is a prerequisite.
  •        National Criminal Record Check
  •        Working with Children Check
  •        Category A Immunisation
