About the Opportunity
As a Level 1/2 Inpatient Social Worker, you will work as part of a collaborative multidisciplinary team, delivering high-quality, trauma-informed, person-centred and culturally responsive care. You will support patients and carers through the emotional impact of illness, assist with complex discharge planning, and help connect people with the services they need to achieve the best possible health outcomes.
This is a diverse and rewarding opportunity where no two days are the same. You will provide evidence-based social work services across a broad inpatient caseload. In this role you will:
- Deliver compassionate, trauma-informed social work services to patients and their families across inpatient settings.
- Conduct psychosocial assessments, provide crisis intervention, counselling and care planning to support positive patient outcomes.
- Collaborate with multidisciplinary teams and community services to coordinate safe, effective discharge and ongoing care.
- Contribute to service improvement, professional development and evidence-based practice within a supportive Social Work team.

About You
We are looking for someone who has:
- Tertiary qualifications in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
- Experience delivering psychosocial assessments, crisis intervention, counselling, advocacy and care coordination, ideally in a clinical setting.
- A strong understanding of the psychosocial impacts of acute illness, chronic conditions and trauma, with a commitment to person-centred, trauma-informed care.
- Excellent communication and relationship-building skills, with the ability to work collaboratively within multidisciplinary teams and with community partners.
- A current Working With Children Check, National Criminal Record Check and unrestricted Driver Licence.
